What is the average amout paid to a software developer for allowing
another program demo to be included ("bundled") with their program? 
In other words, what is the average amount paid per downloaded bundled
copy?

You may have problems finding a definitive answer to this question for
a few reasons:

1. Payments made may not always be monetary
2. This is information that the bundlers and developers would not want
to disclose for obvious reasons.
3. There is no central authority that requires developers to report
this type of information.
