The poem is called "Poem for Everyman", by John Wood, from "How Do You
Feel? A Guide to Your Emotions" (1974).

There is one error in that page's text of the poem: the two lines
"carved their way into / my heart" should actually be divided so that
"heart" is the sole word of the second line. Also, the text should be
lined up with the left margin, rather than centered. But the lines
otherwise appear to be accurate, according to the sources cited below,
especially the Christopher Day book, which I suspect is more accurate.
(Even the space before the very last line is the same in the
Christopher Day book.)
