I need to get a quote for a customized business pad holder -see

It's made by Hazel but I can't find their website or number.  I want
to talk with a sales rep about the feasibility of putting 5 logos and
company names on the pad (instead of the usual one logo.)  I'm
interested in 300-500 units.

I'm NOT interested in other styles or brands of pad holders- Only Hazel.

Since GA Researchers usually ask...I'm located in Florida but our
business office is in Washington state.
