I am currently using Dundas Upload
(http://www.dundas.com/Other/index.aspx) on several of my Website
pages.  I need the uploader to upload files for my data and password
recovery service.  I am having a problem that many users are uploading
files and not identifieing themselves even though I request it on both
the upload instruction page and the successful upload confirmation
page.

To put it simply, I need a programmer to give me code and instruction
on how to add fields for name, E-mail address and phone number to the
uploader fields. I need this information to be sent to me by E-mail or
uploaded in a companion text file.
