The American Family Insurance website details three ways of making a claim:

2) Phone - 1-800-374-1111

3) Or "report your claim to your American Family agent", the list of
Phoenix agents can be found by searching for Phoenix here:

Their Phoenix office is located at:

Although their site does not specifically mention a "Claims Office",
it is fair to presume that claims go to agents, and agents pass them
on to the Phoenix office.

The address is for the main American Family Insurance office in Phoenix.

The first answer was good, just not specific enough for my
verification. 2nd clarification is good.  Thank you!
