Hello nomi,

Thanks for using GA.
I posted a similar answer for a "patient satisfaction survey" in GA
and you can find out more about it over here (
https://answers.google.com/answers/main?cmd=threadview&id=21040 ).
I've given many sites in this question along with the price range.

Also, visit this page ( http://www.supersurvey.com/?sezgs1 ). They
have also given the pricing along with the specifications that can be
included in the surveys.

Here is one nice site you can check
http://www.quask.com/

Search terms used:

user survey online: ://www.google.com/search?hl=en&lr=&q=user+survey+online&btnG=Google+Search

I hope this helps. Feel free to ask for any clarification and rate
this answer if you are satisfied.

Regards,
netcrazy

Hi mvguy!
Thanks for your answer.
Did you get the Question Clarification?
We want to do the survey in the US, preferably in California or
another sun state.
The group of people need to be melanoma patients, close relatives of
melanoma patients, other people at high risk for melanoma, people with
suspect skin blemishes .

Do the sites you mention find the appropriate group of people - it was
not clear to me whether we would need to post these surveys on our own
website or whether they would be sent out to some mailing list, or
whether we would need to provide the mailing list. Can you clarify
this?

Hello nomi-ga,

mvguy-ga asked for the clarification and it's me who answered your
question. I think you misunderstood.

Regarding your query, you can create your own survey with whatever
question you want to put for your purpose. Once your survey is ready,
you can send an email to all the users with the link. Check this page
( http://www.supersurvey.com/?sezgs1 ). Go to the right side and read
the "How it works" section. According to them, you can distribute your
survey using automatic system functionality.

Similarly for ( http://www.quask.com/ ) site. They also do the mass
mailing.

I hope this clarifies.

Thanks

http://www.surveyresearchgroup.com/

They are a medically focused data collection firm in Sacramento, CA.

They offer Survey design, Data Gathering, Data Analysis & Report
generation services.

The general cost for conducting most surveys range from $3-20 per
person in general depending on the complexity of questions and method
of communication. These two are directly dependent upon the method of
collection as well.

Hope this is helpful
