"Smeg head" (pronounced "smeg 'ed" by those with a certain variety of
British accent) is a term of derision used on the UK science fiction
comedy show "Red Dwarf." A major character on the show is a hologram
named Arnold Judas Rimmer. Rimmer is considered an insufferable git by
a spaceship technician named Lister. Hence, "Rimmer, you're such a
smeg head."

As for the origins and meaning of "smeg," the word was created by the
show's writers:

"Red Dwarf is famous for innovating the word 'smeg' in order to remove
swearwords from the show and to add to a futuristic terminology. Some
examples of the word in context are 'smeg head', 'smeg off',
'smeg-for-brains', and 'smegging hell'. The writers of Red Dwarf have
stated that they invented the word and that it has no connection with
any similar real words, such as 'smegma'."

To get a bit of the show's flavor, here's a description of one of the
funnier episodes, in which Lister tries to teach an android named
Kryten how to call Rimmer a smeg head:
