Dear auntiega,

ICD9 Code 564.1 stands for the diagnosis: "Irritable colon Colitis:
adaptive membranous mucous Enterospasm; Irritable bowel syndrome;
Spastic colon."

ICD9 Code 820.8 stands for the diagnosis: "Unspecified part of neck of
femur, closed; Hip not otherwise specified; Neck of femur not
otherwise specified."

put into a HCFA form or a UB92 form. ???
