I'm looking for a detailed step-by-step instruction on 1) how to add
three additional fields to Outlook 2002 Meeting Request form; 2) how
to deploy the modified form for the use by entire company.

Hi,

1-) Isn't this just done by choosing "New" in the "Field Chooser"? Do
you have a problem creating the form, or are the extra fields not
appearing when you're sending the form?

2-) The following article describes how to copy over the form from one
computer to another:

Looking for instructions for creation and deployment of the form. The
reason for which I started with the form is that I need to add three
custom fields to each meeting request (e.g., webex meeting number and
password, etc...) I tried to use MS Outlook help but got nowhere.
