One of these companies offers Universal Life, Term Life, Deferred
Annuities, and Group Life.  That same company has annual premiums
around $75 million.  I can't figure out which one of these it is.

I have researched the information you seek using the online
information provided by the Commonwealth of Pennsylvania's Insurance
Department, at the following URL:

Through this public domain service, it is possible to look up the
previous year's reported assets and premiums for any insurance
provider licensed to sell policies in the Commonwealth of
Pennsylvania.

By referring to the company information listed for each of the
companies you mentioned in your Question, I was able to narrow the
list down. First, Fidelity Mutual is currently undergoing
rehabilitation, and is not actively insuring new parties. As well,
Protected Home was recently (Nov. 30, 2003 to be precise) merged into
the National Guardian Life group, meaning that information regarding
its assets and premiums is no longer available separate from the
Nation Guardian Life information.

Of the remaining companies, the one that fits your criteria the best
is Harleysville Life. In 2002, they collected $75,014,965 US in
premiums nationwide, of which $33,314,253 came from Pennsylvania
alone. According the the information from their website, at:
