Hey mjheal-ga,
When you say "middle name," I assume you mean a name she recieved at
birth and not her maiden name, which is Pierce. Here's a link to a
biographical sketch:
http://www.whitehouse.gov/history/firstladies/bb41.html
As to her name at birth, I believe it's safe to assume she had no
middle name. Every reference to her birth I looked at, admittedly a
limited number, referred to her simply as "Barbara Pierce." I searched
in Google on the following terms:
"Barbara Pierce Bush" "middle name"
and found a document that states she indeed has no middle name:

Not the most definitive answer, but hopefully one that will be useful
to you. Perhaps her husband could share; after all, he's got an extra
one ;)

Hey mjheal-ga,
I can find no reason to believe or even suspect that Mrs. Bush has a
middle initial of "J" or any other middle name. In my state (Rhode
Island) and, I expect, in others as well, you can't get a copy of a
person's birth certificate unless you're a family member or have their
permission. You could attempt to contact the town of Rye, New York,
where she was born or else the relevant agency in New York state
(Dept. of Health/Vital Records?), but I don't think you'll get a copy.
indicted-ga notes that "some historians believe" there is a connection
to the tragedy at the Bay of Pigs. Well, to be blunt, I'd seriously
question the validity of this "belief" as well as the reputation of
these "historians."
