Hello, trindigo-ga!

I have found some examples of court reports written by expert
witnesses and forensic psychologists. I have also located a basic
template for an Expert's court report.

I hope these are helpful. Please do not hesitate to ask for
clarification if needed before rating my answer. That way I can be
sure you have the information that you need!

Here is an example of a summary court report written by an expert witness:

You can also download a template for an expert witness court report
from the following link:

"Model Form Notes for Experts Report."
http://www.ewi.org.uk/form_notes.asp

"Model Form Notes for Experts Report:

The Governors of the Expert Witness Institute recommend these guidance
notes and the Model Report which can be *** downloaded here ** follows
for all experts who may be required to prepare reports in civil
litigation. The Model Report may also be used mutatis mutandis for
criminal cases and other tribunals and fora where opinion evidence is
requested.

(Please click on the highlighted text which says "downloaded here.")

(Or scroll to the very bottom of the page where it reads - "Model Form
of Experts Report"
Click here to download a copy of the Model Form of Experts Report in
Microsoft Word format.)

Though I have not copied the entire example, the main points of the
expert witness court report should include the following points:

CONTENTS

SUMMARY OF CONCLUSIONS
INSTRUCTIONS
ISSUES
BRIEF CURRICULUM VITAE
DOCUMENTATION
CHRONOLOGY/CASE ABSTRACT

TECHNICAL BACKGROUND / EXAMINATION OF THE FACTS IN ISSUE
OPINION
LITERATURE/REFERENCES
EXPERTS' DECLARATION
STATEMENT OF TRUTH

		APPENDICES
