Thanks for the links. 

Allow me to clarify. I am looking for questionnaires that will assist
providers of business services to better understand the needs of their
markets.

For example, a number of b2b marketers work on the assumption that
prospective clients purchase to satisfy a particular business needs or
problems. I am looking for examples of questionnaires that would help
to identify these needs and problems and how these prospective clients
might buy and use these business services.

I realise that a generic questionnaire does not exist so therefore I
would be happy with market research questionaires for specific
business services which I can then generalise. In particular, a market
research questionnaire for someone introducing marketing consulting
services would be great. Or a web design agency. Or change management.
