where is james hutchcraft moak m.d.

where can I find Dr. James Hutchcraft Moak and write to him?. He last
practiced  in Oakland, Calif. but he has left that hospital (Highland
hosp)I must find him
right away. He told me that my husband died of a rare bacteria .My
husband left an insurance policy and in order to pay ,the insurance
company wants to write to Dr. Moak about what he knows and where he
got the information about the bacteria. PLEASE help me....Thank You.
My husband died at North Bay Medical Center in Fairfield, Ca. Then dr
Moak practiced at Highland. Now I cannot find him. He graduated from
Ole Miss in 1999 but he is not listed with current alumni.And
Medbd.ca.gov only gives his LAST working address. But he is gone!
Please help.

After he left the Oakland, CA-based Alameda County Medical
Center/Highland General Hospital, Dr. James Hutchcraft Moak assumed a
position as Assistant Professor of Emergency Medicine with the
Vanguard Medical Emergency Physician Group at the University Hospital
of Cincinnati, Ohio.

information on how to contact Dr. Moak in person:

There can, by the way, be no doubt that Dr. Moak is currently employed
at the Cincinnati Emergency Medicine Department. I found him mentioned
in the 2004 Cincinnati University Hospital directory of physicians and
in the clinical shift schedules for January, February, and March 2004.
The short biography in the directory matches exactly with the facts
you provided.
