I am looking for a few PHP scripts that allow me to automate surveys. 
It should support radios, drop-downs, check-boxes, and text-areas.  I
mean "automated" in the sense that the survey is defined in a MySQL
database and automatically generated from that.  The results should
also be saved to a MySQL database.  A plus would be a sort of survey
designer that offers a GUI on top of the MySQL configuration.  Thanks!
