Apparently he sometimes has one-on-one sessions with people who are
featured on his TV show:

"Last year, "Dr. Phil" producers invited viewers to submit videotapes
of why they wanted to lose weight. Marilyn Sorensen and a friend made
a tape that was chosen from 7,000 submissions... Immediately after the
show, Sorensen and the other challengers (three men and nine women)
spent five days at a Beverly Hills mansion where they learned about
healthful eating and exercising and got to know the other participants
and had a one-on-one session with McGraw."

I haven't seen any mention of Dr. Phil having a private practice
anymore. His focus seems to be on show business these days.

Of course, if money is no object, then the answer is almost certainly,
Yes. I suppose the question is really, would Dr. Phil offer
one-to-one coaching at a price you could afford.

What would that price be?
