Head to EPA's Enforcement & Compliance History Online (ECHO) database at:
http://www.epa.gov/echo/
and click on "all data", which will take you to:
http://www.epa.gov/echo/compliance_report.html
On this page, do the following:
--enter the word [ treatment ] in the "Facility Name" box
--select Region 1 from the "EPA Region" pull-down menu (you'll have to
repeat the search for each of the ten EPA regions)
--at the bottom of the form, check the box that says "Include minor facilities"
--click on "Search"
This will take you to a results page that lists (for Region 1) 180+
facilities. At the bottom of the list is a button marked "Download".
Click on this to get a text delimited file that you can then save and
import into a spreadsheet.
Repeat the process for each of the EPA regions, and you'll have your
national list.

EPA's databases definitely contain the names and phone numbers of
contact people at the WWTPs, but as I said earlier, this information
is not readily extractable from their online databases.
You may want to submit a Freedom of Information Act request to EPA.
To do so, just fill out the form at the following site:
http://www.epa.gov/foia/requestform.html
Select "EPA Headquarters" to receive the request, and spell out the
details of what type of list you want (for example, a national list of
all wastewater treatment plants with plant name, address, contact
name, and telephone number). Make sure to specify that you are
seeking an electronic file, rather than a paper printout of a list.
