[GREEN,RED,BLACK WHITE} ON THIS LENS ARE FOR ?
HOW IT IS WIRED AND WHAT THE "ES" IS FOR

Howdy mikes150-ga,

Any idea at all of the model number?  I think I have a wiring diagram
for the lense, but want to make sure.  I am still working on what the
"ES" means in this case.  Thanks!

LOOKING AT THE LENS FACE " COSMICAR TV LENS   ES 12.5mm 1:1.4   made in japan "
looking at the back it has " 0191762 "
looking at the side it has a 1/8 " hole with a flat head screw down in the hole 
AND THAT IS ALL THAT IS ON IT
I DON'T KNOW IF I CAN LOAD A PICTURE IF I I WILL

Howdy mikes150-ga,

I am going to post this as a Comment as it is probably going to
be hard to ascertain the model number, etc. for your lense.

As the Cosmicar lenses are made by Pentax, I am thinking their
wiring is consistent between their lenses and Cosmicar's.

The Ronald A. Massa Associates web site has a Pentax manual in
PDF format that has wiring diagrams for some Pentax lenses,
including one that has the wiring configuration you describe.

Page 36 of that PDF file has the following:

Red - Supply Voltage
White - Video Signal
Green - LEVEL Remote
Black - Ground
