This comes from testimony before the House Committee on Commerce,
Subcommittee on Oversight and Investigations. The testimony, given on
April 6, 2000, is that of Lewis Morris, Assistant Inspector General
for Legal Affairs in the Office of Inspector General (OIG), US
Department of Health and Human Services.
"There are approximately 5,000 third-party billers in the United
States. However, the OIG has just issued a report on computerized
billing systems that incidently raised the issue that HCFA's ability
to identify these companies is limited. Likewise, it is unknown how
many of the approximately 700 million claims per year processed by
Medicare are submitted by third-party billers."

Here is another estimate (date unknown):
"There are 3,000 to 4,000 entities that call themselves third-party
billers," says Brad Lund, executive director of the Healthcare Billing
and Management Association (HBMA), a trade association representing
third-party medical billers. "The vast majority are sole proprietors
working from home, but can also be publicly traded companies with
millions of dollars in annual revenues."
