In the Medicare Program, home health agencies are paid for each
episode of care according to the case mix weight of the patient. 
Agencies have average case mix weights for all of their patients and
states have average case mix weights for all the agencies in their
state.  I am looking for the average case mix weight by state in 2002
and if it were available, in 2003.
