I need a mail form  for my website. I need to make a feedback form.
Please help me. Also, I would like a banner-less mail form.

Can you use free cgi scripts ?

I found the most flexible and easy to use solution to make any kind of
feedback forms for websites for you. This solution does not require
free cgi scripts on your server.
The  PHP FormMail Generator allows you to generate any kind of form 
that can be included on your website, the only condition is that php
is supported by your webhoster.

The PHP FormMail Generator is:
"A tool to create ready-to-use web forms in a flash. Once the form has
been generated, you have a full functional web form. Including error
checking of required fields, email address validation, credit card
number & expiry date checking, multiple attachments sending, and email
auto responding"
and has the following features:
"-Unlimited fields of form 
-All standard type of fields, plus lots of template, such as credit
card number & expiry date...
-List/menu builder for multiple lists of Radio Buttons, Selection Menu, Check Box. 
-Error checking : credit card number & expiry date, email address,
required fields checking.
-Mail multiple attachments 
-Email auto responding 
-Store submitted form data to a text file"

The program is freeware under the GPL license, commercial use however
requires a fee of 15 $ to be paid.

I tested the program on my own server and must say that I was really
impressed by the ease of use. Just upload the unpacked files to your
server, follow the simple instructions given (you will have to allow
the script to write files to your server by doing a chmod ) and
generate any kind if form you want to have.
After the download of the generated files to you local system you can
rename the file as described in the readme file included in the
script:

"How to use this Generator :
  ---------------------------
1. Define fields for your form.
2. What happens after click "Generate" button:
- Attempt to save your fields to a .ini file
-If the .ini saved, try to out put two files : yyyymmdd-xxxx.php,
yyyymmdd-xxxx.lib.php
-If all 3 files created, you will get a pop up window of your form

Now you have a ready-to-use web form
3. Download two files yyyymmdd-xxxx.php & yyyymmdd-xxxx.lib.php
4. Rename file yyyymmdd-xxxx.php to what you want, such as order.php.
   don't rename file yyyymmdd-xxxx.lib.php.
5. Upload that two files to your web site.
6. Done!"

If you don?t want to install the generator script on your server
please post a clarification request and tell me which fields you will
need in your form, I will generate the form for you and post the
necessary data for you here.


This is by far the best solution I know. 
I hope this helps to solve your problem, if anything should still be
