I am looking for a survey tool that has been designed to specifically
facilitate "360 leadership feedback" for high school or college age
youth.  An online collection and reporting method would be ideal.  I
am aware of "fully customizable" 360 survey sites that would allow me
to build my own.  However, I am now looking for a site or service that
already had developed the 360 aimed at school age students.

I have the exact thing you want. Let me tell you what it is and how I
found it. I have a company that can set up a 360 leadership program
directed at youth in your age group you specfied and take away the
hassle of customizing, who provided an "online collection and
reporting method". No fuss, no muss!

I did a google search for "360 leadership feedback youth" Here is the
link to othat search.

And I went to the first link on the page, which was

I went down to the bottom ofthe page, where it gave contact
information and called the # listed there to ask them a few questions.

I then asked the receptionist that answered for someone to discuss 360
feedback questions about their services. She routed my call to
Barbera, who, when I told her who I was and why I was called, asked me
to read your question to her and she would let me know if it could be
done. I told her what you had asked and she said that they would be
able to do all the things I mentioned in the first paragraph of my
answer to you.
