I am designing a health and fitness community/portal application that
we will start to build very soon.  It has most of the features of a
standard community portal, including : forums, chat, individual blogs,
articles and news, etc. In addition, we will build some of our own
unique applications. including fitness program management (tied to a
calendar), personal training quizzes, small group blogs (shared by 3-4
people), and so forth.  In addition, we will need to perform extensive
customizations to the interface to have it match the look/feel of our
brick and mortar products and services.

Rather than build everything from scratch, I am looking for extendable
portal that we can use as a platform.  Here are my questions :

1. Where can I find a list of community portals with the extension
capabilities I'm looking for (open source and paid for product ) ?
2. Where can I find reviews of extensible community portals ?
3. Based on the above, which portals would best suit my needs ?

I haven't decided.  Most likely it will be either
Linux-Apache-MySQL-PHP or Windows with SQL Server.  I'm open.

Well opensourcecms.com is going to be your best friend at this point. 
It basically gives you every open source (free) CMS (content
management system) and allows you to essentially test drive it on
their site by running a copy themselves and providing you the admin
login and password.

They are, IMO, the most informative site when it comes to this type of
thing, simply because you get to try it before wasting your time
setting it up, only to find out it isn't what you're looking for.

Also -- PortalApp for the Windows platform provides the feature you're
looking for. http://www.aspapp.com
