Looking for a form for firemen to fill out for use in notifying family
in case of death on the job

I would assume most fireman keep a form in their personnel file at the
station, however, maybe you're looking for something else? Would you
mind going into more detail?

I am looking for sample forms used by fire companies to keep on file
in case of a death while on duty. The form would be filled out by the
fireman and sealed in an envelope which can be opened only by
authorized personnel if needed. The form would give instruction about
whom to notify, etc.

Dear apostolate-ga;

Thank you for allowing me an opportunity to answer your interesting question.

Being a long-time member of the public safety field myself I can tell
you with certainty that the emergency notification forms used by most
public safety offices (including fire departments) are standard forms
used by the Human Resources or Personnel Department of the city for
whom the person works. In other words, the fire department personnel
use the same standard emergency notification form as the sanitation
workers, police department, clerks or anyone else employed by the
city.

Here are some examples:

This one, from the City of Superior (Wisconsin), is a standard
no-frills form. It looks virtually identical to the one we currently
use at my police department and that our firefighters, animal control
officers and park rangers also use.

This example is a much more thorough and designed specifically for
emergency services personnel. It is used by some public safety
departments in Ohio; in this case the Dublin Police Department (I
think you'll be pleased with this one):

Exactly what I was looking for. I am the chaplain of a large
metropolitan fire and police department. I could not believe that we
did not have the type of form you have found.
