I am a payroll administrator for a small employer in Illinois. I
entered the wrong social security number for one of our employees in
our computer, and now it has gone to the IRS and State
governments, through W-2s, state unemployment forms, and state tax
records, as income attributed to an incorrect SSN.  Now the employee is,
understandably, having trouble getting their taxes filed.  What forms
will I need to file in order to correct this error?
