How are Chinese physicians trained and credentialed?

I am referring to physicians in mainland china.  The answer should
explain whether physicians are "tiered" i.e. the US equivalent of
physicians assistants, primary care practitioners, specialist
practitioners; and how much training they require to achieve that
status, what (if any) ongoing training they require, and how are
licenses maintained.  By "licenses maintained", I mean whether ongoing
fees or training is required to remain a licensed physician.
