I don't know about qos.I know one isp called http://www.quickSP.com 
and it doesn't provide broadband.

This is a very good list of isps from r-t in usa .You can see the
column dialup and click

on the link to check for broadband.The list is huge.Most of T1,T3
provide broadband.
