I am trying to locate the name and phone number of the manufacturer of
a little known, sparsely distributed Jamaican drink called 'Original
Blend Roots Drink.' The address on the bottle is 734 Franklin Avenue,
Suite 344, Garden City, NY 11530. Can you help me?

For what it's worth, the address you've given belongs to a UPS store. 
"Suite 344" is actually just a P.O. box at the UPS Store, not a suite.
 I spoke with a nice person there, and she informed me that, yes, box
#344 did belong to "Original Blend" at one time but it's no longer in
use.
