For what it's worth, I called the publisher, CL Productions, Santa Fe,
New Mexico at 1-800-203-3597.  I was told that the book is no longer
in print.
 
Would you be interested in finding a library that has a copy?

I could direct you to a library that has a copy.  If that's what you
want, let me know your approximate location, so that I can find the
nearest library with a copy of the book.

I get the impression, though, that you're hoping to buy the book. As
such, I wouldn't want to direct you to a library if you'd be unhappy
with that sort of answer.

The problem is that the book is out of print, according to publisher. 
As such, new copies are not available for sale.  I checked various
used book sites and could not find any copies available for sale.
Perhaps another researcher will have more success. Good luck!
