I was provided this info for purchasing meds online. I submitted this
info from my doctor and was told I needed more info...........what
info do I need besides this..........(If needed, what do you accept
for medical records?)

(Medical records that are supplied can not be more than a year old.
They must be signed by a doctor and must contain information about the
condition for which you are requesting medication and/or any lab work
you have had done.) What am I missing ?
