I am looking for doctors or physicians in England who will issue a
prescription for medicine. The information I will give over the
internet as the doctor/physician requires - I do not wish to go there
in person.
I am not looking for a pharmacy to do this prescription for me - it
should be an independent phsician or doctor only. I would like
a list of 4-5 doctors/physicians with as many contact details as
possible (phone, fax, website.email (if any), address).

I very much doubt if any doctor in the UK would give you a
prescription without having examined you.

Why, here in the UK, vets won't even provide treatment for you to give
to your pet, unless they know the animal.

Although the page this is on refers to Botox, it quotes:

e-mail may seriously compromise standards of care where:

The patient is not previously known to the doctor.
No examination can be provided.
There is little or no provision for appropriate monitoring of the
patient or follow-up care.

Doctors who wish to provide telephone or online services should
consider carefully whether such a service will serve their patients'
interests, and if necessary, seek advice from their professional
association or medical defence society.?

There are organisations out there that can provide the service you are looking for

One is called "e-med" they can be found at http://www.e-med .co.uk

The following quote was taken from an interview with Dr. Julian Eden
for the BBC's clickonline program:

"... As long as you get good information coming in about a medical
problem then quite often you can make an accurate assessment and
diagnosis where appropriate for that condition. With us they know they
get a two or three hour turnaround time, urgent problems can be dealt
with immediately and they can get any medication, referral or
diagnosis that they need within a matter of minutes."

According to the report the "Health on the Net Foundation" (
http://www.hon.ch )have a HONcode accreditation for approved medical
websites.
