Depression in Nursing Home Residents
Recognizing and Treating Depression in Dementia in a Nursing Home Population
Steven C. Samuels, MD,[1] of the Department of Psychiatry, Mount Sinai
School of Medicine, New York, NY, provided an overview of the diverse
problem of depression in the nursing home, including difficulties in
diagnosis, undertreatment, and potentially inappropriate treatment.
The focus included use of specific educational programs to enhance
recognition of symptoms of depression in residents with dementia as
well the current failure of the Pre-Admission Screening and Annual
Resident Review (PASARR) system to detect patients with treatable
psychiatric conditions. Issues raised included the problem of both
undertreatment and overtreatment with antidepressant medications, as
some facilities may rely solely on medications to treat any depression
symptoms. Psychosocial stimulation, behavioral therapy, and activity
programs remain vital to the emotional well being of nursing home
residents. Several models of depression education, screening, and
awareness programs were presented that can cost-effectively lead to
improvements in the documentation, recognition, and treatment of
depression in nursing home residents with dementia.

By way of an introduction: paving the way for a psychiatric referral -
The Psychiatric Consultant
"I'm not crazy!" your patient replied when you broached the subject of
a psychiatric referral. Stammering, you replied: "Maybe it would help;
perhaps you could see someone to help you. People can have trouble
dealing with their illnesses. It's tough. Psychiatrists are physicians
with expertise in things like these."
