Hi,

I'm glad that you were pleased with the bar/restaurant questionnaires.

As for a mystery shopper questionnaire for a UK supermarket... I
haven't found anything yet.  It's very late where I'm at (2:45 in the
morning).  I could work on it some more tomorrow, but you might
consider posting that as a new question so that another researcher
might be able to locate something for you sooner.
