Prescription rates dating back to 1995 and including ?certificate?

The perfect answer would include a table with the types of
prescription in a column and the rates on each date that they change
in rows e.g. (made up data)
Type	19/1/02	26/4/03
Prescription	£6.20	£6.50
Bulk prescription	£19.00	£20.20

The web-site owner will be starting vacation on 19th June 2004 so
additional value is created by answering prior to that date.

Hello Davemanchesteruk

The information you require is scattered around the internet but I
have managed to compile it into a table for you.

The certificate prescription data was the most difficult to track down
but can be verified through searches on the BBC website and through
the English and Scottish parliament websites.  If you require a full
list of my sources I would be happy to list them for you.

The standard prescription charge is detailed in a number of places, I
got the information here:

(Note the charges for this go back to 1952 in the table on this page
but a similar table for certificate prescription would be nearly
impossible to track down!).

Here is the information you need:

In each year the prescription charge increased on 1st April.
