You do not say for which country you require costs, or whether state /
public sector or private sector.

Reference costs for the National Health Service in the UK can be had
through documents linked from

The main document is at

Medical costs vary widely from place to place. For example: several
years ago, I had a surgery in Oklahoma, and one month later I had
exactly the same sort of surgery in Ohio. The Ohio procedure cost
nearly five times as much as the one in Oklahoma.

The best source of cost information is checking what Medicare pays.
Most hospitals will base the charge on what Medicare is paying. For
example, some hospitals will attempt to charge 3 times what Medicare
pays to private insurances. However, of course when a Medicare
patient comes, they are charged only the Medicare Price. Go to this
webpage for the 2004 Medicare prices.
http://www.gamedicare.com/FEE2004/index.htm. You will need to know
the ICD-9 code for the procedure you are inquiring about. That can be
searched by typing your diagnosis and "ICD-9" in a general web search.
