I want to find a supplier who will take my tiff/bmp/jpg/paper/whatever
template, and use his machine to print/etch/paint/whatever the
enlarged, to scale image on a blank shower door (glass or plastic).

The image needs only to be a single colour - say white or red - and is
in fact simply a silloutte.

I'm looking for a life-sized silloutte of Kylie Minogue - i've
uploaded it to my website - http://www.smartautomation.co.uk/kylie.jpg

This would look best etched into a glass shower door. I'm not sure a
vinyl stick-on would work terribly well but i might be convinced
otherwise.

Almost any sign writer will be able to cut a design from a computer
file (although vector graphic file would be preferred, from a design
program such as CorelDraw .CDR files) using a "vinyl cutter" onto
self-adhesive vinyl film, which can then be applied to the shower
door. A wide variety of vinyls are available including translucent and
acid-etched glass effect.

This photo would need to be converted into a single-colour silhouette
image (probably by a graphic designer person, rather than a software
program) then be rendered into vector art. A signwriter could however
print the photo onto vinyl on a large-format photoprinter, then cut
round the body outline and give you your silhouette that way.

If you can see the sort of technique you want used on any shop windows
or doors, ask them who did their signs. You want someone local you can
discuss your requirements with and see their samples before deciding.
