A php extension is an extension usually used for a webpage rather than
a image file and PHP files are usually used to create dynamic
websites. It is analogous to .html or .htm. If you open this file in
your text editing software (eg Notepad or Word) you should see what
looks like web page code (I would recommend *against* opening this
file in your web browser).

If you know the sender of the email containing the image I would
recommend contacting them and asking them to resend the image as a
.jpg or .png file. It may be that the sender is trying to direct you
to a page on the internet that contains the image, in which case the
URL would look like http://theserver.com/theimage.php, if this is the
case ask them to send the URL written in the text of an email instead
of as an attachment. You can then visit the page at your leisure in
your web browser.
