I am looking to create a "performance measurement index" based upon
information gathered from a 10 question survey.  What is an "index,"
how is an index built, what are the mathematical underpinnings
necessary for the index to be a valid representation of the data
collected?  My goal is to keep the questions and the index simple and
straightforward, but I want to understand how to best format the
survey and the index for quality purposes.
