Health Insurance Co. of America (located in Plymouth Meeting, PA) has
a Vice President with the same name of the Mr. Miller you mentioned. 
Their website is www.lifehealthamerica.com which you may wish to refer
to for more information about the liquidation and an e-mail contact
for the Statutory Liquidator who may be able to provide you with
information that would answer your questions if you are perhaps a
creditor or stockholder.
