Am trying to locate this physician to request copies of records for
patient.  Could not find Peter I Liber listed in physician category in
Wheaton, nor at this address.  How can I find him?

moved to a new location within the city. This is indicated by the

I assume that the upper address describes the location of Dr. Liber's

So maybe Dr. Liber has changed locations more than once since the time
he was resident in Willow Site. Should the upper two addresses not
prove useful, I suggest that you also try this third one.

I also performed a search using the database of the American Medical
Association (http://www.ama-assn.org). According to them, Dr. Peter
Ihor Liber is still resident in Wheaton, 1701 Shire Court, as
indicated by WhitePages.com:
