Where do recruiting companies (like dice.com/monster.com/hotjobs.com)
commonly get their leads for IT contracting positions?

What I meant is that I would like to know where the RECRUITING
companies get their IT leads (which they then post on Dice, Monster,
Hotjobs, etc).

Dice gets them from placement by recruiting companies 

Monster gets them from placement by recruiting companies

Hotjobs gets most of its adds by spidering and a percentage by placement 

although I do suspect that Dice and monster do some spidering of their own

Lots of those jobs on Monster were placed by the company themselves.

Either the recruiting company is hired by the company who has a job
available or the recruiting company uses cold calling to see if the
company has jobs available and then offers their services

Well established companies like Matrix usually have a large client
base , smaller one and two man companies usually try the cold calling
method
