Hi, my name is Joe. I am currently seeing a doctor in New York City.
My question is, what do I have to do to take possession or have my
medical records transfered to a doctor in New Jersey?

Hi Joe,

The physician in NJ can request your medical record from the physician
in NY, or if you prefer, you may make the request yourself (either
way, you will be the one who will have to pay the fee). Your NY
physician will retain the original and send only a copy (there will be
a charge of no more than 75 cents per page) - you may request your
entire record or just the relevant parts (to save on the cost). You'll
find directions on what you need to do in the following link.

A qualified person shall not be denied access to patient information
solely because of inability to pay.?"

Dear Joe,

I'm sorry to see that my answer wasn't what you were hoping for. It's
not easy to nail an answer on the first go-round with only one or two
sentences to go on, that is why the Clarification option is there -
there is no limit to the number you can post and they can be so
wonderful in helping the researcher find exactly what a questioner
needs. I'm sorry you didn't use it before closing your question - I'm
sure if we had worked together, the results would've been brighter.
