2) How many Cardiac (heart-only) Intensive Care Units are there in the
United States?
3) What certifications by third parties (JHACO, AHQA, HEDIS, etc.) are
there for intensive care facilities?

My guess is the definition and certification is slightly different in
each state  A wild guess is 100,000 cardiac units plus 100,000 units
for other specialties such as pediatrics plus 200,000 general purpose
ICU. This will not be nearly enough, if we have a large scale
nationwide disaster.
