I have been told that there is a medical database containing
personally identifying information and past diagnoses that have been
submitted on claims by an individuals' past and current  insurers over
time that is shared between
medical insurance companies.  How do I access it and check my own
information?  How do I correct it if there are any errors?

There is a MIB, but they are not the Men In Black, but rather a company
that maintains a database that includes "medical conditions represented
by one or more of about 230 codes."

They have a "Request Your Record" page.  You will want to read this page
in detail for the procedures to follow to get a copy of your record.  It
might cost you $9.00, or there might not be any cost, depending on your
circumstances.

Request Your Record - For U.S. Residents

Get a copy of your MIB Record, if any, by following the these steps:

Step 1  Download a Request for Disclosure Form - HTML PDF **
Step 2  Fill in the Request for Disclosure Form completely
Step 3  Print the Request for Disclosure Form
Step 4  Sign and mail the Request for Disclosure Form (along with your
check/money order for $9.00 if not paying by credit card) to:

Once you have received and reviewed your records, you can do the following
to correct it.  From the FAQ page.

"If you question the accuracy of information in our files, you may write
the MIB Information Office (MIB, Inc., P.O. Box 105, Essex Station, Boston,
MA 02112) and seek a correction in accordance with the procedures set forth
in the Fair Credit Reporting Act."

The (long!) details of the Fair Credit Reporting Act (FRCA) can be found on
the U.S. governement Federal Trade Commission (FTC) web site.

Depending on what medical services you have used, you might not even have
