i need the correct 2004 billing codes , to Medicare Florida for
administration and product for flu vaccine,  also do i bill my cost
for the cost of the vaccine itself?  thank u in advance

Hello mdpa173,
Flu shots are filed with the same biling codes nation wide for filing
done after 10-01-03 use hcpcs code 90657 for children 6-35 months of
age , 90658 for individuals 3yrs of age and older, G0008 for the Admin
of flu vaccine and all should be filed with Diagnosis code V04.81. 
When you click on this link it will take you to a page in the cms
website, under publications in the center section is a publication
called 2003-04 Immunizers' Questions & Answers Guide to Medicare
Coverage of influenza and Pneumococcal Vaccinations. The answer listed
