How does a software developer transition from being a developer to a manager?

I define 'manager' loosely to include any position where software developers
or other software development managers report to the manager.  Sample
titles: "software manager", "director of development", "VP of
Engineering".

A suitable answer will include links to a number of sites that discuss
such transitions.  Links should focus on how to get the promotion
although it's fine if some instead discuss the challenges of being a
green software development manager.
