I live in DELAWARE and I am presently on medical leave (12 weeks).  I 
will not be able to return to work within that time frame.  My
employer, also in DELAWARE, is hedging in providing whatever paper
work has to be completed so that I may receive long term disability
insurance benefits.  Long term disability insurance is provided as a
benefit through my employer.  They tell me I should go through Social
Security.  But my LTD insurance book says I'm entitled to 60% of my
wages for 5 years.  Can I insist that they provide me the paperwork so
that the long term disability insurance will start when my sick and
vacation pay expires in 12 weeks?
