I am looking for the total size of the US commercial video market. 
The market specifically includes:

1. The amount of money spent on DVDs, both rental and sales
2. The amount of money spent on VHS tapes, both rental and sales
3. The amount of money spent on cable and satellite subscription fees
4. The amount of money spent on pay-per-view and on-demand fees (including hotels)
5. The amount of money spent on movie tickets
6. The amount of money spent on Internet video downloads
7. The amount of money spent on advertising within these venues

The numbers should be for 2003.  They should all be in US $.  I would
need numbers for each line item and sources for each.
