I need to move my 92 year old father from Southern Pines, North
Carolina to Indianapolis, Indiana.  He is not bed ridden, but very,
very feeble.  I was hoping to hire an ambulance service to move him. 
I would like to be able to ride in the ambulance with him.  I think
the distance is about 650 to 700 miles.  Does such an service exist -
and what would be the cost?

Yes there are ambulance services like these and they are usually done
by air. Would a list of air ambulance services be ok with you instead
of the usual land travel?

I believe my good colleague easterangel-ga is correct in stating that
much medical transport is carried out by air.  But there has recently
been an upsurge of ground transportation services as well, such as the
services provided by Ameritrans:

possibilities for you to choose from if you want ground or air medical
transport services. Special thanks extended to my colleague

medical transport services.
 
Prices will depend not only on the miles but also on the other needs
and equipment your patient will need so it will be best to ask them
for the specific quote since the cost will depend on the various needs
of different patients.

Transport by air ambulance is significantly cheaper and more
comfortable than transportation via traditional ambulance.  You will
find many private ambulance services provide "Long Distance
Transports", but the bill is usually thousands of dollars for a trip
of 600-700 miles.  Ambulance cots, even the best ones, aren't very
comfortable.  I can't even imagine a patient having to be on one for
that duration of time.  Travel by air is the way to go.

iminquisitive-ga is right about how much more comfortanle and faster
an air ambulance charter is compared with ground transportation, but a
650-700 mile trip would most likely cost you between $5,000-7,000.
Don't limit your search to air charter companies in North
Carolina,also look for air charter companies in  Indianapolis since
the charter aircraft needs to reposition back to it's home base after
the flight. And don't forget to call an air charter broker like,
http://www.airbrokernet.com. They can usually find you a cheaper air
ambulance flight or a one way flight special.
