Usually yes, but it would be best to ask your doctor (or whoever conducts the exam).

Caffeine can probably throw you system out of wack more than any food can.

It is considered fasting as long as you don't use cream/milk or sugar.
The effet of caffiene is another issue not related to the need to fast.
